Beware of Fake IRS Tax Bill Notices
The Internal Revenue Service and its Security Summit partners are warning taxpayers and tax professionals of fake IRS tax bills related to the Affordable Care Act.
The IRS has received numerous reports of scammers sending a fraudulent version of a notice- labeled CP2000 - for tax year 2015. The issue has been reported to the Treasury Inspector General for Tax Administration for investigation.

The current Human Resources System (HRS) has been upgraded and replaced with a new web-based system, Banner Human Resources (Banner HR). As a part of Banner HR, Web Time Entry (WTE), a web-based pay period time entry system, must be utilized. Banner WTE is used by employees to collect time worked.
Beginning July 16, 2016, all employees are expected to use the Banner HR Web Time Entry System. To enter time, employees will need access to an updated web browser, such as Internet Explorer, Firefox, Chrome, or Safari. Employees are still required to submit a paper leave form to their supervisor.

It applies to the health information that is protected by HIPAA that is generated or maintained by the Institution’s Employee Health Plan.

Student/Hourly Exit date
Beginning July 1, 2014 (FY15), it will be the Department Supervisor’s responsibility to submit a Student Worker Exit Date form (attached) to the Human Resources (HR) Department when a student/hourly employee is no longer working for the department.
In order to maintain up-to-date employment statuses and keep accurate records, the HR department must have an ending date for OPSU employees. Therefore, every supervisor must inform us when a student/hourly employee is no longer working at OPSU.

Students and vendors are responsible for checks received from OPSU. A $15 fee will be charged for reissuance of any lost or stolen checks.

Absence Request/Report Form (Paid Leave Forms) Update
There is a new look to the Personnel Leave form to adhere to OTRS policies for sick leave used (OPSU’s Sick Hours). (Attached is the Revised form)
The top of the form you still put your name, office, and CWID.
An employee requesting time off would select if the time off is for the employee being sick (#1), vacation/personal leave (#2), or Administrative Leave (#3).
Next the employee would put the date(s) absent and total hours. The employee can add a remark but it’s not required.
The employee and supervisor must sign the Absence Request/Report Form. After the employee and supervisor have signed the form, it needs to go to the President’s Office.
2% Payroll Tax Expires
The payroll tax cut that reduced the employee share of social security payroll tax from 6.2% to 4.2% has been allowed to expire. Starting in January, workers will once again have 6.2% of their wages up to $113,700 withheld to pay for Social Security, up from the 4.2% rate that’s been in effect for the past two years.
