Anyone withan opsu.edu email
- What are some common reasons for which a form may be submitted?
Changes in behavior,frequent absences, unwillingness or inability to progress academically, a general feeling that something isn't quite right, etc.
- What happens after the form is submitted?
The forms arerouted to the Dean of Academic Resources where the appropriate next steps are established.
This may be contact with the Coordinator of Counseling Services, tutoring services, mentoring services, the BIT, etc.
The student will be contacted and encouraged to come into the Academic Resource Center.
